mothers2mothers (m2m) is an Africa-based NGO that unlocks the potential of women to eliminate paediatric AIDS and create healthy families across ten sub-Saharan African countries. m2m trains, employs, and helps empower women living with HIV to work as community healthcare workers in understaffed health centres and underserved communities. Job Description

The purpose of this position is to provide clinical technical expertise in the development and implementation of interventions and strategies at site, district and sub-district level to Improve patient HIV/TB outcomes in the various program areas. Provide Direct Service Delivery support to the beneficiaries and their caregivers as and when required. To strengthen the HIV testing services, care and support by ensuring that newly diagnosed and referred patients are linked to facilities and necessary services for care, To ensuring that patients are retained in care, tracking and tracing of lost to follow-up and re-engage them to care.

Key Performance Areas:   

Linkages and Community Engagement 

  • Ensure that patients diagnosed within the community are linked to care and commenced on ART as applicable
  • Ensure that clients who have been referred to other services are linked with these services
  • Follow up with facility staff to ascertain if client has been received at the facility
  • For clients who have not been received at the facility: 1. Follow up with the client telephonically and arrange for them to come in to the facility. 2. For clients who still do not present to the facility (post-telephonic communication)-handover client details back to Family Mentor
  • Conduct community profiling and map referral systems of individuals between health facilities, communities, and social service providers
  • Work with operational managers in health facilities and social service providers to ensure that the services provided meet the demand created
  • Strengthen linkages between the community and the facility and improve linkage to the care system with a stronger focus on case finding and client indexing
  •  Assist facilities and social service providers to develop systems to enhance the linkage into care of clients referred from community
  • Facilitate the tracing of defaulters in conjunction with Community Health Workers at testing events
  • Engage communities to improve the quality of care, retention in care and adherence to treatment

Monitoring and Evaluation 

  • Ensure receipt of all reporting forms from site staff
  • Ensure complete and accurate data entry and follow-up on missing forms or information
  • Maintain filing system to ensure secure and organised storage of site reporting forms and other M&E documents
  • Maintain accurate data and compile reports for project staff and stakeholders
  • Complete M&E tools as required
  • Assist in running of data reports to make decisions
  • Records relating to daily work performance e.g. number patients successfully referred, number of beneficiaries received a direct service,  feeding of Linkage  tracking tool
  • Update progress reports on tracing of patients

Prevention 

  • Support and work with district Coordinators conduct Community Dialogues
  • Ensure and Support that each district maintain accurate and complete standardized data collection and reporting tools for all national required Condoms Distribution indicators.
  • Support and strengthen districts to have reliable stock out supply of Condoms then provide onsite training to family mentors
  • Support and strengthen community demand creation, Outreach, health promotion and campaigns etc. plans and implementation to ensure correct data collection and reporting.

General departmental support 

  • Compile and submit monthly reports
  • Attend relevant training and updates.
  • Collaborate with any research activities as necessary
  • Participate in all relevant meetings e.g. facility staff meetings, sub-district meetings, district meetings, provincial meetings for department of health, department of social development and department of education
  • Other duties as assigned by line manager
Job Requirements

Qualifications and Experience:
•    A Diploma/ Degree in Nursing (SANC Certificate & Registration)
•    3-5 years’ experience in Health programmes
•    2 years’ experience in a Technical Assistance role (Training, mentoring and quality improvement Initiatives and data quality mentor)
•    NIMART certificate, PC 101 trained
•    Essential knowledge of ART adherence and counselling (Advanced adherence counselling HIV management training certificate)

Competencies:

•    Excellent attention to detail and deadlines (Report writing)
•    Strong interpersonal skills and ability to build relationships with stakeholders and clients
•    Good problem-solving abilities and ability to work independently and prioritize competing demands
•    Excellent written and verbal communication skills
•    Passion for working with communities
•    Commitment to uphold the Patients’ Right Charter and promote the principles of the Batho -Pele
•    Good listening skills and empathy towards to others
•    Ability to facilitate partners and stakeholders meetings
•    Strong interpersonal skills and good team player
•    Responsible and mature in dealing with sensitive psychosocial issues
•    High level of integrity
