Inherent requirements of the position:

  • Knowledge of HIV/AIDS, TB and STI
  • Clear criminal record
  • Valid Code 08 Drivers licence is mandatory
  • Experience as a community health facilitator will be advantageous
  • Possess good knowledge of HIV/AIDS prevention, care and treatment, PMTCT and TB/HIV services
  • Proficient personal computer skills including electronic mail, record keeping, and routine database activity Internet software; Spreadsheet software and Word Processing software
  • Excellent attention to detail and deadlines
  • Strong interpersonal skills and ability to build relationships with stakeholders and clients
  • Good problem-solving ability
  • Excellent written and verbal communication skills
  • Very good reasoning ability

Duties and Responsibilities include but not limited to:

  • To link newly diagnosed HIV positive people to a facility or a NIMART so that they can initiate the client on ART
  • Liaise with DOH facility staff and Pepfar funded partners to overcome barriers for ART initiation
  • Strengthen linkages between the community and the facility
  • Assist facilities to develop systems to – enhance the linkage into care of patients referred from community testing events – facilitate the tracing of defaulters in conjunction with the Community Health Worker’s
  • Provide administrative support to ensure that patients receive the appropriate care at each visit
  • Work closely with the District Coordinators to ensure linkage support aligns to community activations
  • Engage communities to improve the quality of care, retention in care and adherence to treatment including supporting
  • Manage community profile and mapping referral systems of patients from the community to facility and vice versa
  • Assist with obtaining and collecting HIV positive client level information for the Index Tracing modality from health facilities and/or Pepfar funded partners
  • Assist with obtaining consent for Index Tracing and when needed assist with Index Tracing
