EDUCATIONAL REQUIREMENTS | – Senior Certificate (Grade 12) or equivalent – HIV/AIDS Counselling Certificate – minimum 10-day Course, which is not older than 2 years from the date of issue or HIV/AIDS Counselling Certificate – minimum 10-day Course, older than 2 years’, but not more than 3 years’, with a certificate for a 2-day refresher course from an accredited provider.

- Ability to work non-traditional hours which will include weekends
Duties and Responsibilities include but not limited to:
- To link newly diagnosed HIV positive people to a facility or a NIMART nurse so that they can initiate the client on ART; this position will be for someone that is not afraid to work irregular working hours
- Liaise with DOH facility staff and district support partners to overcome barriers to ART initiation
- Liaise with the client to schedule clinic appointments for ART initiation and offer transport when needed – an indemnity, waiver and release form needs to be completed before any client gets in the CareWorks or staff member’s vehicle
- Daily reports of all clients linked to care and on those cases were attempts were made but not yet linked to care
- Work closely with the team leaders/lay counsellors to ensure linkage support aligns to community activations
- Manage community profile and mapping referral systems of patients from the community to facility and vice versa
- Assist with obtaining and collecting HIV positive client level information for the Index Tracing modality from health facilities and/or district support partners
- Assist with obtaining consent for Index Tracing and when needed, assist with Index Tracing
- Escalate any challenges to the provincial manager or Programme Manager, timeously
- From time-to-time conduct community-based combination HIV prevention initiatives including STI’s, TB, HTS, risk reduction, interventions in TB, STI’s screening and referrals to high risk sexually active individuals
- Participation in staff meetings and appropriate training workshops
- Ensure timeous submissions of workplans
- Providing HIV Testing Services to clients in line with NDOH policies and guidelines when needed
