Job title: Community Health Worker (CHW)

JOB SUMMARY
The CHW supports the health providers such Medical doctor and nurses through an integrated approach to care management and community outreach. As a priority, CHW activity will promote, maintain, and improve the health of patients and their family. Provide social support and informal counseling, and provide services such as first aid, and blood pressure screening.
Key Duties and Responsibilities
- Provide health information through community outreach, group health education sessions, individual encounters and home visits to assess patient needs and concerns as they relate to their family, their community and their health
- Visiting homes to determine health situations and dialogue with household members.
- Identifying and sensitizing communities on health interventions.
- Identifying common ailments and minor injuries at community level.
- Collecting health data at household levels for analysis.
- Distribution of information education communication materials, mosquito nets and other commodities to households.
- Maintaining and updating health data using health registers.

QUALIFICATIONS
- Minimum qualification of Diploma or equivalent in public health, nursing, social science and/ or in community development.
EXPERIENCE AND SKILLS
- Experience in primary health care, community work or related field.
- Knowledge of and experience in maternal and child health, nutrition, and protection.
- Good communication skills both in Somali and English
- Knowledge and experience on safe motherhood practices, and advocacy.
Child Safeguarding: The position interacts with children and hence requires utmost compliance when dealing with children
